All plans include access to Dr. Corinne’s expert care through in-person and telemedicine visits. All memberships require a 12-month commitment.

Level One: Basic Membership

$75 per month. Perfect for those seeking basic wellness support and preventive care.

Membership Benefits:

Medication Refills: Convenient access to necessary prescription refills
Basic health monitoring and screening
Email access to physician with 24-48 hour response time
In-person visits and follow-up calls
Basic lab results reviews

This plan is ideal for individuals seeking consistent access to quality care with a straightforward, affordable structure. Email responses will be provided within 2 days (text messaging not included).

Please Note: This membership level does not include specialist referrals or coordination with outside providers. Patients requiring specialist care are responsible for scheduling their own appointments.

Level Two: Premium Membership

$100 per month. Ideal for those managing ongoing health concerns and seeking comprehensive wellness support.

Membership Benefits:

Includes everything in the Basic Membership, plus:

Medication Refills: Ongoing access to prescription refills (excludes controlled substances).
24-hour email response time
Text message access during business hours
Convenient Visits with flexible, priority scheduling

In-Person Visits: For personalized care
Phone Visits: Convenient and accessible consultations

Lab and Test Review: Comprehensive discussion and interpretation of lab results and diagnostic tests
Chronic disease management support
Weight loss support
Stress management consultation
Hormone optimization and monitoring

This plan is designed for those who value comprehensive support and accessibility, especially for busy lifestyles or complex medical needs.

Pain Management Membership

$200 per month. Specialized care for patients requiring ongoing pain management and controlled substance prescriptions.

Membership Benefits:

Includes everything in the Basic Membership, plus:

Controlled Substance Management: Prescription and refill management for pain medications following best practice guidelines
Required quarterly follow-up visits for medication monitoring
Comprehensive pain management documentation
Coordination with pain management protocols

Legacy Physicians follows established best practice guidelines for pain management and controlled substance prescriptions. This includes regular follow-up appointments, medication monitoring, and appropriate documentation as required by state and federal regulations.

Please Note: This membership level does not include specialist referrals or coordination with outside providers. Patients requiring specialist care are responsible for scheduling their own appointments.

Concierge Level: Exclusive Membership

$250 per month (Limited Spots Available). Comprehensive care for those requiring intensive support and frequent check-ins.

Membership Benefits:

Includes everything in the Premium Plan, plus:

Unlimited Visits: In-person or phone consultations tailored to your needs
Medication Refills: Quick and seamless prescription management including controlled substances when medically appropriate
Direct Communication: Text and email access for personalized care
Personalized Diet & Activity Plans: Custom health and wellness strategies
Daily Availability: Exclusive access to care 7 days a week, including after-hours support
Specialist Referral Coordination: Assistance with referrals to specialists when needed

Initial Visit

Initial Visit: $295 (60-minute consultation)
Following your initial consultation, choose one of our membership plans.

IMPORTANT NOTES

12-Month Commitment Required: All membership plans require a 12-month commitment. Membership levels cannot be changed mid-term to accommodate temporary needs.
No Emergency Care: This practice is not an Emergency Department or Urgent Care. For emergent care needs, please visit your nearest Emergency Room.
Medicare: Legacy Physicians cannot accept Medicare patients.
Controlled Substances: Patients requiring controlled substance prescriptions must enroll in the Pain Management Membership ($200/month) or Concierge Level ($250/month). Legacy Physicians follows best practice guidelines for pain management and narcotic prescription refills, including required quarterly follow-up visits. Enrollment in any membership level does not guarantee the prescribing or continuation of controlled medications. All prescribing decisions are made solely based on medical appropriateness and clinical judgment.
Specialist Referrals: Level One and Level Two memberships do not include specialist referrals. Patients at these levels are responsible for scheduling their own specialist appointments.
